Everything you need to know before submitting an application on the website for free implantation from Dental Help UA

Colleagues, patients, and their relatives — we have collected all the necessary information in one article, which will definitely be useful before submitting an application for free implantation through the Dental Help UA fund.

Who Can Become a Patient

Before filling out the patient form, choose one of the criteria:

• Military personnel who were direct participants in combat operations since the beginning of the Russian invasion (2014) and sustained maxillofacial injuries;

• Released prisoners of war;

• Military personnel with severe injuries sustained in combat, confirmed by a medical commission.

All the above categories of patients must be able to stay in the rear for at least three months to undergo full treatment and rehabilitation.


What You Need to Submit an Application for Free Implantation at Dental Help UA

• Choose a clinic that will start the veteran’s treatment, using the map available on the website, or contact a dentist in the city where the veteran is located to get confirmation of readiness to perform implantation within the framework of the fund’s initiative. The clinic must work with Straumann and/or Neodent systems and be ready to participate in the “Return a Smile to a Hero” program.

• Prepare scanned copies of the required documents.

Required Documents for Free Implantation

Mandatory for all patients:

• Taxpayer Identification Number (RNOCPP, previously TIN);
• Military ID;
• Certificate of participation in combat operations (UBD) or a document confirming participation in combat.

Mandatory depending on the chosen criterion:

• Certificate from the military medical commission with information about the injury and classification as severe;
• Certificate of maxillofacial injury;
• Document confirming time spent in captivity.

Application Limit — 75 applications per month

This is the maximum number of patients the fund can process and direct to treatment.

The limit is often reached on the first day due to the large number of patients in need. Це пов’язано з великою кількістю пацієнтів, які потребують лікування. The fund makes every effort to direct more veterans for treatment, but some waiting time may be unavoidable.
